The Patient Navigation Network

The Community Health and Social Services Network (CHSSN) recently launched the Patient Navigation Network (PNN), an initiative that aims to improve access to health and social services for English-speaking communities across Quebec.

Supported by funding from the Secrétariat aux relations avec les Québécois d'expression anglaise (SRQEA), the network establishes Patient Navigators in communities across the province to help English-speaking individuals navigate the healthcare system, access services in English, and connect with the resources they need.

As part of this provincial initiative, the Montérégie West Community Network (MWCN) is proud to have a dedicated Patient Navigator serving the Montérégie West region. This ensures that English-speaking residents have local support to help them understand the healthcare system, connect with appropriate services, and access the information and resources they need close to home.

What Does a Patient Navigator Do?

  • Provide personalized support based on each client's unique needs.

  • Guide clients through Quebec's health and social services system.

  • Assist with appointments, healthcare forms, and  understanding medical information.

  • Act as a liaison between clients and healthcare  providers to improve communication.

  • Provide accompaniment and emotional support when  appropriate.

  • Identify and helping overcome barriers to care,  such as language, transportation, financial, or cultural challenges.

  • Refer clients to community organizations and healthcare resources that best meet their needs.

  • Advocate for English-speaking clients and collaborating with healthcare professionals to improve access to care.

  • Maintain confidential client records and following up to ensure continuity of care.

support every step of the way.

A Patient Navigator CANNOT:

  • Transport clients to or from appointments.

  • Provide interpretation or translation services.

  • Diagnose medical conditions or provide medical advice.

  • Prescribe or recommend medications or treatments.

  • Give legal or financial advice.

  • Make healthcare decisions on a client's behalf.

  • Guarantee appointments, services, or outcomes.

  • Access or share personal health information without the client's consent.

  • Provide counselling, emergency intervention, or personal caregiving.

  • Complete clinical assessments or perform medical procedures.

Meet Your Patient Navigator

Hello! My name is Dania James, and I am proud to serve as the Patient Navigator at the Montérégie West Community Network (MWCN). My role is to help English-speaking residents across the Montérégie West region connect with the healthcare services, programs, and resources they need.

My journey into this role has been shaped by years of working directly with people. Before joining MWCN, I worked as a Personal Support Worker, where I witnessed firsthand how difficult it can be for individuals and families to navigate healthcare, especially when language barriers, unfamiliar systems, or unexpected health challenges arise. Those experiences inspired me to continue serving my community in a different way, by helping people find answers, access services, and feel supported every step of the way.

Whether you're trying to understand available healthcare services, searching for community resources, or unsure of your next steps, I'm here to help make the process a little less overwhelming.

Every conversation reminds me why I chose this path. I genuinely believe I have found my calling. Being able to support individuals, empower them with information, and help them access the care they deserve, is a privilege I don't take for granted.
